      The newly released MAIB Annual Report 2024 offers another detailed look into incidents involving UK vessels. While the overall numbers remain relatively stable, the underlying themes, especially those linked to human factors, feel all too familiar.

      We continue to see the same types of issues: man overboard, falls from height, training gaps, procedural drift, and fatigue. These are not new concerns, and yet they persist.

      It makes one wonder, are we truly learning, or simply recording?

      This year’s report doesn’t point to new or emerging risks but rather reinforces how deeply rooted some systemic issues remain. It’s a timely reminder that policies, checklists, and training alone can’t drive safety, not without human-centred design, open culture, and real engagement at all levels.
